The screening takes place in the office of Dr. Warren Hagan, located at 1132 Broadway (located in the Homebank building), on May 3 from 1-4:30 PM. To begin your registration, click "register" in the upper right corner of this page!
Registrants will be seen one at a time, in five minute intervals, so please select a time you are certain you will be able to arrive on time for.
